Ayurveda does not describe PCOS as a single disease.
Instead, it understands it through disturbances in Artava (the menstrual and reproductive tissue) and blockage in the channels that support it.
Depending on how it shows up, PCOS may resemble different classical patterns.
This refers to reduced, delayed, or scanty menstruation. If cycles are very long, irregular, or flow is minimal, Ayurveda sees this as Artava not performed. The tissue is there, but it is undernourished or poorly supported
This means absence of menstruation. In many PCOS cases, periods may stop for months. Ayurveda explains this not as disappearance, but as obstruction.
The downward-moving energy responsible for menstruation called Apana Vata, becomes blocked. When its movement is disturbed, the cycle cannot flow naturally.
A condition described in classical texts where menstruation happens and the body may appear healthy. But conception becomes difficult.
This closely resembles many modern PCOS cases. Periods may come, but ovulation may not be timely or strong enough to support conception.
Granthi refers to cyst-like growths formed due to stagnation in the channels. When digestion and metabolism weaken, a state called Mandagni. The body produces incomplete metabolic residue (Ama).
Over time, this can accumulate and block the reproductive channels (Srotorodha), leading to fluid-filled cystic formations.
So What Is Actually Happening?
In simple terms, Ayurveda sees PCOS as a progression. Mandagni (weak digestion and metabolism), leads to Ama (metabolic buildup). Which causes Srotorodha (blockage in reproductive channels), resulting in Avarana of Apana Vata (disrupted menstrual movement)
This may appear as Artava Kshaya (scanty or delayed menstruation), Nashtartava (absence of menstruation), Pushpaghni Jataharini (menstruation present but difficulty conceiving), or Granthi (cyst-like growths due to stagnation). So instead of describing PCOS simply as “hormonal imbalance,” Ayurveda sees it as a disruption in the nourishment and flow of Artava. Not a failure of the body, but an obstruction in its natural processes.
PCOS and Infertility

Does PCOS Affect Fertility? Yes. Does It End It? No.
Ayurveda makes an important distinction:
